According to United Airlines refund policy, you can take the following steps if you cancelled your flight for whatever reason and now want a refund. Additionally, you must complete out the United refund form in order for the airline to issue you the appropriate refund amount.

Go to United Airlines' Refund page.
Please include your contact information and travel information.
Give a rationale for the refund, but only up to 1,000 characters.
Add supporting documentation to your statement.
To be contacted further, enter your email address and press "Submit."
After filling out the refund form, the refund will be processed within a few days.
